Common Peptide Side Effects: What to Watch For

Side Effects Are Real

Online peptide communities sometimes downplay side effects, but any bioactive compound that produces physiological effects can also produce unwanted ones. Being informed about potential side effects is not alarmism — it is basic risk management. The severity and frequency of side effects vary enormously by peptide type, dose, individual physiology, and product quality.

Common Side Effects by Category

Growth Hormone Secretagogues (CJC-1295, Ipamorelin, MK-677)

  • Water retention: Puffy face, swollen fingers, tighter rings and shoes. Caused by GH-mediated sodium retention. Usually subsides after a few weeks or with dose reduction.
  • Increased hunger: Particularly pronounced with MK-677 (which is technically not a peptide but a GH secretagogue). Can be intense in the first few weeks.
  • Tingling/numbness: Carpal tunnel-like symptoms in hands and wrists. Related to fluid retention and nerve compression. Usually dose-dependent.
  • Fatigue and lethargy: Some users report daytime drowsiness, particularly with evening dosing.
  • Elevated blood glucose: GH antagonizes insulin action. This is the most medically significant side effect and requires monitoring via bloodwork.

Healing Peptides (BPC-157, TB-500)

  • Injection site reactions: Redness, minor swelling, or itching at the injection site. Usually mild and transient.
  • Nausea: Occasionally reported, typically mild and dose-related.
  • Headache: Some users report headaches in the first few days.
  • Dizziness: Infrequently reported, may be related to blood pressure changes.

GLP-1 Agonists (Semaglutide, Tirzepatide)

  • Nausea: The most common side effect, affecting 40%+ of users. Usually worst during dose titration and improves over time.
  • Vomiting and diarrhea: Related to slowed gastric emptying and GI receptor activation.
  • Constipation: Paradoxically, some users experience constipation rather than diarrhea.
  • Fatigue: Reduced caloric intake and metabolic changes can cause significant fatigue.
  • Muscle loss: A serious concern — significant weight loss from GLP-1 agonists includes lean mass, not just fat.

When to Seek Medical Attention

Seek immediate medical attention for severe allergic reactions (difficulty breathing, facial swelling, hives), severe abdominal pain (potential pancreatitis), chest pain or heart palpitations, vision changes, or any symptom that feels dangerous. Do not try to "push through" serious side effects. When in doubt, stop the peptide and see a doctor.
